Of course he knew, they probably asked his permission before they made the mold of his face. The real question is whether he gets a cut of the ticket money. Next time just take a pic with it and send it to his office, see what happens. My buddy did that with a statue of his old gym teacher and the guy thought it was the funniest thing ever. Worst case you get a funny story out of it, best case your dentist thinks you're a little weird but in a harmless way.
