Listen, I used to shrug off missed checks thinking it was just red tape. But seeing a hose burst once changed all that for me. lisa_morgan is right about how dangerous skipped hydraulic checks can be. To report it without drama, go straight to your site safety officer and just state the facts. They care more about fixing the problem than blaming people.

What did I do when my supervisor missed a pressure valve check? I filled out a near-miss report and left my name off it. That way, safety looked into the pump station without pointing fingers. They found a cracked gauge that could have blown. The foreman got retrained, and no one got fired. Just stick to the facts on the form, and let the system work.

Skipping the hydraulic line inspection before a pressure test is insane. That's how you get a fitting blowing off at 3000 psi and cutting someone in half. I've seen a loose coupling whip around and take out a safety glass window twenty feet away. Your foreman is playing with live ammunition, not just skipping paperwork.
